It is notified for the information of all concerned that schedule for submission of admission forms of following programmes for Annual Examination, 2019 is as under:-

Last date to submit admission forms with single fee: 15-11-2019

Last date to submit admission forms with double fee: 22-11-2019

Exam fee rates:

DPT, DHND & DMLS	Rs. 755/-	Rs. 15000/-
Pharm-D	Rs. 7000/-	Rs. 14000/-

The admission forms can be downloaded from here. The fee must be deposited in any branch of Habib Bank Limited in account No. 00427991796403 on computer generated bank challan available here. Demand draft, banker’s cheque and hand written bank challan etc. will not be acceptable

If any candidate / principal deposits less than the prescribed fee. He/she shall have to deposit 03 times of the less/remaining fee otherwise roll no. slip shall not be issued to him/her.

The date of commencement of examination will be notified later on.
